Ready Steady Go: Implementing a Transition Programme

Ready Steady Go is a pioneering transition programme for patients with long-term conditions introduced at Southampton University NHS Hospitals Trust. Led by Dr Arvind Nagra, Consultant Paediatric Nephrologist and Clinical Lead for Transitional Care at University Hospital Southampton NHS Foundation Trust (and the lead clinician for Ready Steady Go) this in-depth interactive workshop gives participants the chance to really get to grips with how the programme works, and to come away with the tools and strategies they need to implement it in their own Trusts.
